Daniel Ho has chord charts in his Secrets of Contemporary Slack Key Guitar book.

Or you can make you own chord chart(s) with charting tools like "ChordHouse" (google it). Just put in your string tunings (you can ignore the lower two strings of the guitar chart for Uke), and then click on the chord(s) desired. Use print-screen to cut and paste into something like MS Word and you are on your way to having customized charts for any tuning.

perhaps a little late to chime in...
go to
www.danielho.com
The newest version of his book has almost all the same stuff as,
"Contemporary slack key", which is OOP.
It does not have a cd, but is recorded on
Hawaiian Slack key guitar, the complete Collection.

The first "Contemporary SLG' besides having a great study cd with practice tips, had a piece for uke called "Haiku" in GDEA ...unusual, but charming, and to me, a bit of twisted violin...(GDAE)....
Kilauea is not traditional...
the 6ths on the first two strings may be good for you--if you survive...
Also, there is not alternating bass, and other differences from traditional SKG...
but his teaching about music theory, history, and practicing aspects is superlative. He also uses classical guitar left-hand fingering and numbering which is logical, easy to read and maybe good for me if I practice.
The fingerings when the left hand shifts/moves resemble what a violinist might do...
although some of us use lone-wolf fingerings...
Hey, Segovia did it...but of course he had great reach...
Some of us have to be lone wolves...
just some of the wolves howl better...
Hey, we can't all be rich and famous...Miss Piggy got the last of those contracts.
