Yep, this topic is pretty well covered. My one thought is, it seems like "ignorance" is a big contributing factor to why these sorts of comments/situations arise. I live in an area (Connecticut) so far from Hawai'i that people here are *clueless* about Hawai'i. They only think of stereotypes (Don Ho, Tiny Bubbles, Honeymoons, etc.) They have so little to go on, so little experience, and so little inclination to research anything because they likely will never be in Hawai'i... I can understand their "distance" from another culture, but if they do venture to say something about it, it would help if they would read up a little bit about it...
I know of a local who thinks that to "educate" people where he can, is what he can do in the face of these sorts of comments/situations. I think that is one constructive way to handle it.
